Tulsa Is One of America’s Worst Allergy Cities. Here’s Why.

Tulsa has landed near the top of a national ranking.

Unfortunately, this is one nobody wants to celebrate.

The Asthma and Allergy Foundation of America ranked Tulsa as the third-worst city for seasonal allergies in 2026, up from fourth place in 2025. Only Boise and San Diego ranked higher this year. Tulsa even finished ahead of Wichita, a longtime contender for the top spot. 

AAFA’s ranking considers annual pollen levels, over-the-counter allergy medicine use, and access to allergists.

But what makes Tulsa in particular so bad?

After practicing allergy and asthma medicine for decades, here’s why Tulsa ranks so high and what we can do to improve it:

Tulsa sits beside a major pollen-producing region

Tulsa lies near the western edge of the eastern deciduous forest.

That puts us where wooded terrain meets the Great Plains. Oak, elm, and maple trees release pollen during spring. Grasses follow as temperatures rise, while ragweed takes over in late summer and fall.

Tulsa also has a long growing season.

Plants have months to flower and release pollen. Strong Oklahoma wind carries that pollen across neighborhoods and sometimes across state lines. Eastern red cedar pollen, for example, can travel into Oklahoma from Texas.

Tulsa’s pavement, rooftops, and parking lots make the problem worse. These surfaces retain heat, which can extend plant growth and encourage plants such as ragweed to produce more pollen.

So removing the oak tree in your front yard will not solve the problem, as pollen is already blowing in from nearby streets, parks, pastures, and counties.

And in Oklahoma, that heavy exposure can cause more problems after pollen season ends.

Ragweed starts a cycle that can last into winter

Tulsa’s ragweed season usually begins around the middle of August.

Sneezing, congestion, drainage, and itchy eyes often follow. Cold weather eventually stops ragweed from releasing pollen, but it does not always end the patient’s symptoms.

Months of inflammation can swell the nasal lining and block normal sinus drainage. Mucus becomes trapped, pressure builds around the cheeks or forehead, and a sinus infection may follow.

That is why a patient may walk into my office on December 15 and say, “My allergies are still terrible.”

By then, pollen may no longer be the main problem. The patient could be dealing with: 

  • Sinusitis
  • Lingering inflammation
  • A respiratory virus
  • Or nonallergic rhinitis

Taking another antihistamine will not fix a diagnosis that was wrong from the beginning. These conditions make Tulsa difficult for allergy sufferers, but pollen alone does not explain our national ranking. 

So why do other high-pollen cities rank lower than us?

Tulsa does not have enough allergists

A city’s allergy burden depends partly on whether residents can receive appropriate care.

Tulsa falls very short in that area.

Many allergy clinics already carry heavy patient loads. Tulsa also lacks an allergy fellowship program that continually brings newly trained specialists into the community. Physicians often remain near the hospitals where they complete their training, which gives cities with fellowship programs a recruiting advantage.

Sadly, this shortage extends beyond Oklahoma.

Congress capped the number of residency positions supported by Medicare through the Balanced Budget Act of 1997. Although hospitals can fund additional positions through other sources, the payment caps continue to affect how many physicians the system can train and where those physicians practice. 

Tulsa is experiencing the consequences, meaning patients have to heavily rely upon something far less effective.

Urgent care treats the flare, not the pattern

When specialist appointments are difficult to obtain, patients often turn to urgent care.

That may help during a severe flare, but urgent care was built to handle individual episodes. Seasonal allergies require continuity.

A patient might visit three clinics over four years and receive prednisone each time. Unless someone reviews the entire record, no clinician may notice the repeated steroid exposure. The patient gets temporary relief while the allergic disease remains uncontrolled.

Repeated systemic steroids can contribute to

  • Elevated blood sugar
  • Bone loss
  • Cataracts
  • Mood changes
  • And other complications

The risk only grows as exposure accumulates.

So while the health systems collect much of this information, the data often sit in separate encounters rather than producing an automatic warning.

Treating each flare as an isolated event keeps patients moving through the same cycle every year, which feeds into another problem…

Tulsa residents are buying relief that often fails

To seek relief for the longer allergy seasons, Tulsans are trying to manage symptoms themselves by buying large amounts of over-the-counter allergy medicine.

Many are not succeeding.

I frequently ask new patients who take a daily antihistamine, “Are you still having symptoms?” They answer yes. Yet they continue buying the same medication because they assume allergies are simply part of living in Oklahoma.

They may also use the wrong treatment. 

In controlled studies, corticosteroid nasal sprays generally relieve nasal allergy symptoms better than oral antihistamines. Technique matters, though. If the spray runs down your throat, very little medicine remains where the inflammation is located.

Other patients do not have allergies at all. A deviated septum, nasal polyps, sinusitis, a respiratory virus, or nonallergic rhinitis can produce similar congestion.

You cannot select the right treatment until someone identifies what is actually causing the symptoms (hence the need for more physicians).

What Tulsa should do next

So, what can we do to not be the 3rd worst city for allergies?

We can start first with improving how we measure and treat the problem.

Tulsa needs a reliable local pollen-counting station, more allergy specialists, and medical systems that flag repeated urgent-care visits and steroid prescriptions. Accurate pollen counts would help residents plan outdoor work, exercise, and medication around high-exposure days.

For patients, the advice is simpler: if your medication is not controlling your symptoms, stop repeating the same failed routine.

Allergy shots may provide longer-term relief by gradually making the immune system less reactive to ragweed, grass, oak, and other allergens. They require testing, medical supervision, and consistent follow-up, but they can help patients move through Tulsa’s worst pollen weeks without losing control of their schedules.

It will take a lot of work, but if enough people advocate and push for these changes, there’s a future where Tulsans can live and breathe much more easily.
